Pre-Participation Physical Form

Before participating in athletics, every potential athlete must have an active physical on file. A physical is currently good for TWO years (as of the 20-21 school year). The link to the physical form is listed below. You can turn this into the HS office. Hard copies will be available in the HS office as well.
